第一節(jié) 施工現(xiàn)場(chǎng)質(zhì)量管理檢查記錄表

    該表是GB50300—2001第3．0．l條的附表，健全的質(zhì)量管理體系的具體要求。一般一個(gè)標(biāo)段或一個(gè)單位（子單位）工程檢查一次，在開(kāi)工前檢查，由施工單位現(xiàn)場(chǎng)負(fù)責(zé)人填寫(xiě)，由監(jiān)理單位的總監(jiān)理工程師（建設(shè)單位項(xiàng)目負(fù)責(zé)人）驗(yàn)收。下面分三個(gè)部分來(lái)說(shuō)明填表要求和填寫(xiě)方法。

      一、表頭部分

    填寫(xiě)參與工程建設(shè)各方責(zé)任主體的概況。由施工單位的現(xiàn)場(chǎng)負(fù)責(zé)人填寫(xiě)。工程名稱(chēng)欄。應(yīng)填寫(xiě)工程名稱(chēng)的全稱(chēng)，與臺(tái)同或招投標(biāo)文件中的工程名稱(chēng)一致。

    施工許可證（開(kāi)工證X填寫(xiě)當(dāng)?shù)亟ㄔO(shè)行政主管部門(mén)批準(zhǔn)發(fā)給的施工許可證（開(kāi)工證）的編號(hào)。建設(shè)單位欄填寫(xiě)合同文件中的甲方，單位名稱(chēng)也應(yīng)寫(xiě)全稱(chēng)，與合同簽章上的單位名稱(chēng)相同。建設(shè)單位項(xiàng)目負(fù)責(zé)人欄，應(yīng)填合同書(shū)上簽字人或簽字人以文字形式委托的代表一工程的項(xiàng)目負(fù)責(zé)人。工程完工后竣工驗(yàn)收備案表中的單位項(xiàng)目負(fù)責(zé)人應(yīng)與此一致。

    設(shè)計(jì)一單位欄填寫(xiě)設(shè)計(jì)合同中簽章單位的名稱(chēng)，其全稱(chēng)應(yīng)與印章上的名稱(chēng)一致。設(shè)計(jì)單位的項(xiàng)目負(fù)責(zé)人欄，應(yīng)是設(shè)計(jì)合同書(shū)簽字人或簽字人以文字形式委托的該項(xiàng)目負(fù)責(zé)人，工程完工后竣工驗(yàn)收備案表中的單位項(xiàng)目負(fù)責(zé)人也應(yīng)與此一致。

    監(jiān)理單位欄填寫(xiě)單位全稱(chēng)，應(yīng)與合同或協(xié)議書(shū)中的名稱(chēng)一致�？偙O(jiān)理工程師欄應(yīng)是合同或協(xié)議書(shū)中明確的項(xiàng)目監(jiān)理負(fù)責(zé)人，也可以是監(jiān)理單位以文件形式明確的該項(xiàng)目監(jiān)理負(fù)責(zé)人，必須有監(jiān)理工程師任職資格證書(shū)，專(zhuān)業(yè)要對(duì)口。

    施工單位欄填寫(xiě)施工合同中簽章單位的全稱(chēng)，與簽章上的名稱(chēng)一致。項(xiàng)目經(jīng)理欄、項(xiàng)目技術(shù)負(fù)責(zé)人欄與合同中明確的項(xiàng)目經(jīng)理。項(xiàng)目技術(shù)負(fù)責(zé)人一致。

    表頭部分可統(tǒng)一填寫(xiě)，不需具休人員簽名，只是明確了負(fù)責(zé)人的地位。

    二、檢查項(xiàng)目部分

    填寫(xiě)各項(xiàng)檢查項(xiàng)目文件的名稱(chēng)或編號(hào)，并將文件（復(fù)印件或原件）附在表的后面供檢查，檢查后應(yīng)將文件歸還。

    l、現(xiàn)場(chǎng)質(zhì)量管理制度。主要是圖紙會(huì)審。設(shè)計(jì)交底、技術(shù)交底。施工組織設(shè)計(jì)編制審批程序。工序交接、質(zhì)量檢查評(píng)定制度，質(zhì)量好的獎(jiǎng)勵(lì)及達(dá)不到質(zhì)量要求處罰辦法，以及質(zhì)量例會(huì)制度及質(zhì)量問(wèn)題處理制度等。

    Supervision units filling out the unit name, should be the name of the contract or agreement in line. Column should be the chief supervision engineer contract or agreement clearly responsible supervision of projects, supervision units can also be clearly documented person in charge of the project supervision, supervision of qualified engineers must have a certificate, professional counterpart.
