第一節(jié)   施工現(xiàn)場質(zhì)量管理檢查記錄表
該表是GB50300—2001第3.0.1條的附表，健全的質(zhì)量管理體系的具體要求。一般一個標(biāo)段或一個單位（子單位）工程檢查一次，在開工前檢查，由施工單位現(xiàn)場負責(zé)人填寫，由監(jiān)理單位的總監(jiān)理工程師（建設(shè)單位項目負責(zé)人）
　　驗收。下面分三個部分來說明填表要求和填寫方法。
　　一、表頭部分填寫參與工程建設(shè)各方責(zé)任主體的概況。由施工單位的現(xiàn)場負責(zé)人填寫。
　　工程名稱欄。應(yīng)填寫工程名稱的全稱，與合同或招投標(biāo)文件中的工程名稱一致。
　　施工許可證（開工證），填寫當(dāng)?shù)亟ㄔO(shè)行政主管部門批準(zhǔn)發(fā)給的施工許可證（開工證）的編號。
　　建設(shè)單位欄填寫合同文件中的甲方，單位名稱也應(yīng)寫全稱，與合同簽章上的單位名稱相同。建設(shè)單位項目負責(zé)人欄，應(yīng)填合同書上簽字人或簽字人以文字形式委托的代表——工程的項目負責(zé)人。工程完工后竣工驗收備案表中的單位項目負責(zé)人應(yīng)與此一致。
　　設(shè)計單位欄填寫設(shè)計合同中簽章單位的名稱，其全稱應(yīng)與印章上的名稱一致。設(shè)計單位的項目負責(zé)人欄，應(yīng)是設(shè)計合同書簽字人或簽字人以文字形式委托的該項目負責(zé)人，工程完工后竣工驗收備案表中的單位項目負責(zé)人也應(yīng)與此一致。
　　監(jiān)理單位欄填寫單位全稱，應(yīng)與合同或協(xié)議書中的名稱一致。總監(jiān)理工程師欄應(yīng)是合同或協(xié)議書中明確的項目監(jiān)理負責(zé)人，也可以是監(jiān)理單位以文件形式明確的該項目監(jiān)理負責(zé)人，必須有監(jiān)理工程師任職資格證書，專業(yè)要對口。
　　施工單位欄填寫施工合同中簽章單位的全稱，與簽章上的名稱一致。項目經(jīng)理欄、項目技術(shù)負責(zé)人欄與合同中明確的項目經(jīng)理、項目技術(shù)負責(zé)人一致。
　　表頭部分可統(tǒng)一填寫，不需具體人員簽名，只是明確了負責(zé)人的地位。
　　二、檢查項目部分填寫各項檢查項目文件的名稱或編號，并將文件（復(fù)印件或原件）附在表的后面供檢查，檢查后應(yīng)將文件歸還。
　　1、現(xiàn)場質(zhì)量管理制度。主要是圖紙會審、設(shè)計交底、技術(shù)交底、施工組織設(shè)計編制審批程序、工序交接、質(zhì)量檢查評定制度，質(zhì)量好的獎勵及達不到質(zhì)量要求處罰辦法，以及質(zhì)量例會制度及質(zhì)量問題處理制度等。
　　2、質(zhì)量責(zé)任制欄，質(zhì)量負責(zé)人的分工，各項質(zhì)量責(zé)任的落實規(guī)定，定期檢查及有關(guān)人員獎罰制度等。
　　3、主要專業(yè)工種操作上崗證書欄。測量工、起重、塔吊垂直運輸司機，鋼筋、混凝土、機械、焊接、瓦工、防水工等建筑結(jié)構(gòu)工種。
　　電工、管道等安裝工種的上崗證，以當(dāng)?shù)亟ㄔO(shè)行政主管部門的

Supervision units filling out the unit name, should be the name of the contract or agreement in line. Fill in the header section can be uniform, without specific staff signature, but clearly the responsible person position.
Second, check the check portion of the project completed project file name or number, and documents (copy or original) is attached to the back of the table for inspection, inspection of documents should be returned.
